General Secretary Speech

Speech of the General Secretary

It is my immense pleasure, proud and privilege to welcome you all from Bangladesh Society for Peritoneal Dialysis (BSPD).

It is the demand of the present time and situation to let the people and the physicians, especially renal physicians to know and learn about the impact and advantages of CAPD on patients suffering from End Stage Renal Disease (ESRD) at an affordable cost and convenience.

Chronic Kidney Disease (CKD) is increasingly recognized as a global public health problem. The number of patients suffering from ESRD is increasing exponentially with the epidemics of diabetes mellitus, chronic glomerulonephritis and hypertension. You will be surprised to know that the annual growth rate of ESRD is far beyond the world population growth rate. In 2017 the annual global growth rate is 1.12% and that of ESRD is around 5%.

About 20 million people are suffering from different forms of kidney diseases in Bangladesh and 40 thousand patients are dying every year. Only 10% of the people who are suffering from ESRD in Bangladesh can afford treatment, as the treatment is very expensive. Seventy five percent of the patients stop hemodialysis within 3 months due to lac of financial inability. By the time the family has spend all the deposits and sold all their properties. So when he dies the whole family is at ruins. All these cruel realities have given birth to this Bangladesh Society for Peritoneal Dialysis (BSPD). The aim of this society is to aware about a more affordable less expensive modality of treatment Continuous Ambulatory Peritoneal Dialysis (CAPD) among people, doctors and renal physicians, train young nephrologists and nurses in this field.

More than 40% patients on Haemodialysis are unaware of CAPD. There are many inherent advantages of CAPD over Hemodialysis. Better preservation of residual renal function. More liberal fluid and fruits intake. The quality of water used in CAPD is much purer than what is used in Haemodialysis. No chance of hyperkalemia. Little dietary restriction. Patients are less anemic as no blood loss unlike in haemodialysis there is 10-15 ml of blood loss in each session i.e., 120-180 ml per month. No chance of hypotension and sudden death. No need of vascular access. No need of heparin. Better blood pressure control. Less chances of hospitalization. Better for travelers. Less chances of HBV and HCV contamination. No need of infrastructure. Less manpower i.e., for 100 patients, 4 nurses for CAPD versus 40 nurses for haemodialysis is required. Considering all these advantages “PD first” policy has been adopted in many countries like Hongkong and Thailand. Spain, Taiwan, China, India, Mexico, Canada and US have PD favored policy. And I believe that this society will broaden the scope of CAPD to be a good modality of renal replacement therapy in our country.

I hope that this annual convention and scientific conference will be a great success. Once again, I welcome you all and thank you all for your kind presence.
